(There  is very famous Dhanwanthari  temple in a place  called Nelluvai  in Kerala .It is believed  that the Aswini devas  consecrated this temple  .In ancient days for any Ayurvedic doctor to start practice in Kerala  , he has to  do BHajanam in  this temple fopr 41 days(He has  to be in the temple morning to evening and eat only the prasda given by the temple , This temple is 21  km from Guruvayur.  the  writing in sanskrit is done by Sri V.N.Devarajan .He is 80 years hailing from Vallioor in Tirunelveli Distt. settled in Delhi since 1958. Learned Krishna Yajur Veda from 1964 to 1970. He retd from United India Insurance Co.in 1999. My hobby is to transliterate by hand from Grantha Lipi and Malayalam Lipi into Devanagari and Tamil as my handwriting is fairly good, as many younger Pandits are more familiar with Devanagari. He does  not perform Homa or Puja, but participate in homams at temples only or recitations like Rudram, Upanishad, Acchidram etc.)

   The God holds Conch, Chakra, pot of nectar   and a leech  . 

SAlya is a word for paddy (Nellu in Malayalam) ,. Mukha is word for moth(Vai)  and so  Salya Mukha   can be translated as  Nelluvai) 

1,Bishagwara  Rema kanthaa  Saali vakthra   puraalaya

Raga rogaadhi bhootham maam  pahi  Dhanwanthare  hare

Oh Doctor, consort  of Lakshmi, one   who has  temple in Nelluvai

Please save me    from the devil of   diseases and Passion, Oh God Dhanwanthari  , Oh Hari

2.Sankha chakra, sudha kumbha jalukadhara  Madhava,

Karunakara  maamaasu paahi Dhanwanthare  Hare

Oh Madhava   carrying   conch, Chakra and leech,

Oh merciful one  , please protect me fast, Oh Dhanwantari  , Oh Hari

3.Akhilaadhaara govinda, nikhilaarthi  nivaraka,

Vyakulam maam Maha vishno , Pahi Dhanwanthare Hare

Oh Govinda   who supports the entire universe who  cures  all  sickness,

Protect me from my worries , Oh Vishnu  , Oh Dhanwantari  , Oh Hari

4.Shyama varna  SAdananda, Saalithunda  pureswara,

Vipannam maam  Viswa bandho paahi Dhanwanthare hare

Oh black coloured   forever joy , Oh Lord   who lives in Nelluvai

Of relative  of the world, I am ruined, Protect me  oh Danwanthari  , oh Hari

5,Janardhana  , Jagannadha, jalajaaksha   jwarapaha

Pranthaardra  prapannam maam, Pahi dhanwanthare Hare

Oh lord who excites people, Oh lord of universe, Oh lord  with lotus like eyes who cures fever

I am praying and sudffering devotee, Protect me  Oh Dhanwantari  , Oh Hari

6.SAnatha gunaaraama, sarvaamaya  kulanthakaa

Rujaarthi maam Kruparase, Pahi dhanwanthare  hare

Oh perennial   storehouse  of good qualities, who destroys all those  with evil ,

I am suffering from sickness oh merciful one, Protect me  Oh Dhanwanthari  Oh Hari

7.Saantha saali mukhaagaaraa, saradhendhu  nibhanana

Sarma  Sagara  niswam maam, Pahi Dhanwanthare  hare

Oh Lord in whom peace dwells forever, who looks like  the autumn sun,

I am suffering from sickness  of skin, Protect me  Oh Dhanwanthari Oh Hari

8.Sudhasana  pathe, viswa  klesa nasana  kesava

Prathaptham maam  sudhaa pane, paahi dhanwanthare  Hare

Oh Lord who reached  for nectar, Oh kesava who destroys suffering of the world,

Oh Lord holding nectar , I am greatly suffering  , protect me  , Oh dhanwanthari  Oh Hari

9.Vasudeva   hrishikesa vyaadhi jatha  vinasaka,

Vana malina varaakam maam , paahi   Dhanwanthare  hare

Oh Vasudeva, Oh Hrishikesa, who destroys the getting of disease,

Oh lord wearing forest garland, I am pitiable, protect me , Oh dhanwanthari  Oh Hari

10.Narayana  Dhaya sindho nadha  Saali mukhalaya

Padmanabha  murare maam pahi  Dhanwanthare Hare

Oh Narayana , Oh ocean of mercy, Oh Lord  of Nelluvai  temple

Oh Padmanabha , oh killer of Mura, protect me Oh Dhanwanthari, Oh Hari

11,Rogaathuraanaam     abhaya pradhaayin Daakshinyavaardha   bhishageesa  murthe,

Kalyana Rase, Kamalapathe maam, Dhanwanthare pahi  Hare  Murare

Oh god who gives protection   to those who suffer  from disease, Oh doctor God filled with pity,

Oh Lord  granting welfare, Oh lord  of Lakshmi, Oh Dhanwanthari protect, Oh Hari  , oh killer  of  Mura

12.Ksheerabdhi nadha ,  dhara  Chakra   Sudhaa , jalukaa dharin, Dhayardhre,

Shubha Saalimukhalayesa, sarvamayaapaha bhishagwara,

Santhaam maam   Dhanwanthare pahi  hare  murare

Oh lord oh ocean of milk who carries  Chakra  , nectar  and the Leech  , Oh lord who melts with pity,

Oh   good god of Nelluvai  oh God , remove all my   sickness

And always   protect me  , Oh Dhanwanthari, Oh Hari  , oh killer of Mura
